Individualized Support

Assisting our clients one on one during their visits to the health Department, including providing quality interpretive services.

The Community Health Worker team works with the Nursing and Clinical Services division at the Health Department to provide the following to our clients:

• in-person interpretation for individuals who prefer to speak Spanish during their appointments
• support in accessing cost-feasible preventative screenings, including application completion, submission, and tracking
• referrals to other resources as needed
